Families with an annual income of ₹8,00,000 or below are eligible. Required documents for verification:

- Ration card / BPL card
- Valid government-issued photo ID

- Confirmation via SMS/Email along with digital EWS Family Card.

How to Avail
- Apply for EWS Family Card (online form + upload documents).
- Receive Approval SMS/Email and your Digital EWS Card.
- Book OP appointment and present your card at billing.
- Discounts auto-applied to eligible services.
EWS-Facility Family Card (Privilege Card)
- Household-linked digital card with unique ID & QR code.
- Lifetime validity (re-issuance available in case of loss).
- Cardholder privileges: OP discounts, health camps, priority desk services.
- Misuse or non-eligibility may lead to cancellation.
Terms & Conditions
- Discounts apply to OP services only (consultations, diagnostics, day procedures, ward charges as listed).
